My sister is away from 3+ months of training for work. She sent her daughter to stay with a friend for the first month and the next two months or so she'll be with me. This weekend I have her though because the friend is away with her family. The friend informed me my niece arrived to her with a bad diaper rash but that it had improved with cream my sister sent. My first time changing her diaper I was taken aback it's a nasty looking rash and I've never dealt with one like it, I have a 2 yr old and 1 yr old so not that much experience ....It's super bright red mostly around the labia with raised blisters. I put corn starch on it that time to dry it some and it was a poop diaper but I didn't dare wipe it with wipes so I did a sink bath. Today she pooped a lot and I did a baking soda bath in the sink and slathered on the zinc oxide cream, the blisters seem to be open now...I feel so bad for her I want to just help it heal as quickly as possible. Please share any other tips that may help heal this up quick I know there's diaper free time , she is 16 months old. If it's not cleared up by the time I get her permanently I will be likely taking her to ER

I would definitely take her into her pediatrician or even urgent care if you are able. At this point it sounds more like a bacterial, yeast, or allergic rash vs a normal one and you will most likely need a prescription.
- Bacterial we use a Mupirocin prescription.
- Yeast, AF triple paste, Lotrimin, Nystatin
Or doctor might give a steroid cream as well.
(Some previous advice from our doctor) Since the blisters are open, it would be better to switch to something healing. Think Aquaphor, Vaseline, coconut oil, etc. Zinc is good for keeping the area dry, but LO most likely needs a balm of some sort to heal first.

One of my boys gets pretty bad rashes when he poops a lot back to back, sometimes even blisters and he doesn’t sit in a diaper long. Basically what helps him is no wiping only rinsing if he poops, pat dry, we put nyostatin and seal with aquaphor, always keeping the diaper dry to changing back to back and usually by the next day it’s pretty much healed.

When my daughter had a bad rash, the nurse practitioner suggested “magic butt paste” which was a mixture of equal parts Desitin, Malox, and Lotrimin. It really helped.

this combination worked for my daughter too. I use about 50/50 aquaphor baby and extra strength desitin and then about a thumb tip lomatrim and a capful of miralax. Mix it together well and apply generously with every diaper change. Try to change diaper extra often.

If she has blisters she needs to see the pediatrician. It could be a yeast infection or even a reaction to her formula/milk or food she’s eating.

Agree with previous posts that it could be infected at this point. Get her to a pediatrician and see what they recommend. In the meantime try using a 50/50 combo of Vaseline and pure zinc diaper cream & put it on heavy! I would try not to wipe her (I know extremely difficult with poops) maybe wash her and then let her air dry....poor girl. We have also done oatmeal baths (check to see what age you can give those).

I would take her to a doctor at that point. It sounds like a yeast rash to me but if it’s as bad as you say it is I would just go ahead and take her to be seen so you can know for sure and get it treated asap.
